Online Enrollment Directions
- Download and review our enrollment packet so you are familiar with our policies and requirements. You can download an enrollment packet by clicking here.
- Download and complete the Enrollment form found below.
- Click Submit at the bottom of page 4. This will prompt you to save, be sure that you save it where you can find it easily.
- Using your e-mail provider, you will attach the form and e-mail to fcaeast@yahoo.com with a subject line of “Enrollment”. Please note that if you do not attach the form to an e-mail, we will not receive it.
- Once FCA East receives your forms, we will send a bill via PayPal, please note that your enrollment is not official until the payment has been received through PayPal.
